SFL Form 4E – Master Player Roster – This form is filled out by the individual who is designated by the club as the SFL Club Representative. The first work sheet contains the instructions on how to complete the Roster Data work sheet.
This form is an Excel spread sheet. Be sure to use your browser’s “SAVE AS” feature to save this file to your computer. Instructions on how to do this for Internet Explorer are shown below. In Netscape, you simply put your cursor on the link and right click the mouse and select the SAVE THIS LINK AS . . . option.

SFL Form 3 – Players No Longer Playing Travel
This form is filled out by the player, coach, and is approved by the individual who is designated by the club as the SFL Club Representative. See Section II.A.1. of the Season Rules for additional information. (See viewing note below)
Viewing Note: Internet Explorer, at least version 4.72.3110, does not properly load the
Adobe Reader for these files. If you would like to review these files, then use the following
- Put your cursor on the document that you would like to review.
- Right click your mouse and select “Save Target As…
- A “Save As” Dialog Box will appear.
- Select the file location where you would like to save the file (e.g., a floppy disk).
- Open up My Computer (or Windows Explorer) and go to the location where you saved the
file and double click the file that you would like to review. Adobe Acrobat should come up if
you have it loaded on your system.
These files could be viewed directly with several different versions of Netscape when the
Adobe Reader had been installed.
