Electronic Forms - Generally Used By SFL Club Reps.

Image saying New SFL Form 2E - Electronic Team Roster This form is filled out by the individual who is designated by the club as the SFL Club Representative. The first work sheet contains the instructions on how to complete the Team Roster work sheets. This form is also discussed in Section II. of the rules. The rules can be found on the SFL Documents page.

This form is an Excel spread sheet. Be sure to use your browser's "SAVE AS" feature to save this file to your computer. Instructions on how to do this for Internet Explorer are shown below. In Netscape, you simply put your cursor on the link and right click the mouse and select the SAVE THIS LINK AS . . . option.

Electronic Club Contact Information Form This form is filled out by the individual who is designated by the club as the SFL Club Representative and is used to provide the club contact information needed. An Email is sent to the SFL Club Representative and other individuals from the clubs who register teams on how to complete this form.

Electronic Team Registration Form This form is filled out by the individual who is designated by the club as the SFL Club Representative and is used to provide the team contact and other team information needed to register a team. An Email is sent to the SFL Club Representative and other individuals from the clubs who register teams on how to complete this form.

Spread Sheet for Regular Season Field Availability This form is filled out by the individual who is designated by the club as the individual that is authorized to provide the field availability information. This spread sheet is completed as part of the team registration process and provides the standard field availability information that is used for schedule generation.

This spread sheet is completed as part of the team registration process and provides the tournament fields that are expected to be supported by club.

Spread Sheet for Field Availability to Reschedule Games This form is filled out by the individual who is designated by the club as the individual that is authorized to provide the field availability information for games that need to be automatically rescheduled by the SFL.

SFL Form 3 - Players No Longer Playing Travel This form is filled out by the player, coach, and is approved by the individual who is designated by the club as the SFL Club Representative. See Section II.A.1. of the Season Rules for additional information. (See viewing note below)


Viewing Note: Internet Explorer, at least version 4.72.3110, does not properly load the Adobe Reader for these files. If you would like to review these files, then use the following procedure:

  • Put your cursor on the document that you would like to review.
  • Right click your mouse and select "Save Target As...
  • A "Save As" Dialog Box will appear.
  • Select the file location where you would like to save the file (e.g., a floppy disk).
  • Open up My Computer (or Windows Explorer) and go to the location where you saved the file and double click the file that you would like to review. Adobe Acrobat should come up if you have it loaded on your system.
  • These files could be viewed directly with several different versions of Netscape when the Adobe Reader had been installed.
